This form needs to be completed in full in order to make a presentation on an Agenda Item or Topic at a Community Forum. The form must be submitted to Council by 12pm on the day before the Community Forum.
Council requires anyone interested in attending and/or addressing Council at the Community Forum to read and accept the terms contained within the Public Forum section of Council's adopted Code of Meeting Practice prior to attending.
- Clause 4.4 - To speak at a public forum, a person must first make an application to the Council in the approved form. Applications to speak at the public forum must be received by in full prior to 12pm the day before the date on which the public forum is to be held, and must identify the item of business on the agenda of the Council meeting the person wishes to speak on, and whether they wish to speak 'for' or 'against' the item. Submissions of questions or statements on general issues not related to the meeting's agenda items may also be accepted, following these guidelines.
(a) - The approved form will enable identity verification to ensure person/s submitting are not using an alias or other means of identity masking.
(b) - If a person cannot attend in person or via the available audio-visual facility, their approved submission will not be read out on their behalf.

- All requests received will be evaluated prior to acceptance. At a minimum the information provided should include the following:
- Name
- Address
- Contact Number
- Subject Matter
- If they are speaking ‘for’ or ‘against’ the item
Where the above criteria is not met, your question may not be accepted for the forum.

The information collected on this form may or may not be recorded in the Community Forum Record of Proceedings. Regardless of whether the information is recorded in the Community Forum Record of Proceedings, the information shall be stored on Council's Electronic Data Management System.
